Buttock augmentation by fat transfer (BBL) involves liposuction of one or more donor sites — abdomen, flanks, lower back — and meticulous transfer of the harvested fat into the gluteal region. Dr. Saeed performs the injection in the subcutaneous plane only, never below the gluteal fascia, in line with the multi-society safety advisory.

Who is suited to a BBL

Patients seeking gluteal projection or shape correction who have sufficient donor fat. Very thin patients are not always good candidates — donor fat may be insufficient.

Subcutaneous-only fat transfer

The procedure, step by step.

Fat is harvested at low pressure with VASER assistance, processed by closed gravity sedimentation, and injected through blunt cannulas in fanning passes — subcutaneous plane only. Injection is monitored with portable ultrasound to confirm depth.

    Liposuction of the planned donor sites, with the operating table prepared for prone repositioning.

    Closed gravity sedimentation separates viable adipocytes from infiltrate and oil.

    Patient repositioned to prone for the gluteal injection phase.

    4. Subcutaneous injection

    Fanning passes in the superficial subcutaneous plane only. Depth confirmed with intra-operative ultrasound.

    Access ports closed; compression garment fitted; BBL pillow issued in theatre.

Week 1 – 2

No sitting

No direct pressure on the buttocks for two weeks. Prone or side-lying for sleep. Walking encouraged.

Week 2 – 4

BBL pillow

Sitting only with a BBL pillow that off-loads the buttocks. Lymphatic drainage continues.

Week 6

Light cardio

Light cardio resumes. Full sport at week eight.

Month 6

Final volume

Some absorption of the transferred fat is expected — typically 25 – 35%. The remaining graft is permanent.

Fat embolism

The principal historical risk of BBL was fat entering the gluteal venous system. We mitigate this by injecting only in the subcutaneous plane, with ultrasound guidance, and at low pressure.

Fat resorption

25 – 35% of transferred fat is reabsorbed. We slightly over-correct to allow for this.

Asymmetry and contour irregularity

Both donor and recipient sites can develop minor irregularities; touch-ups are possible at six months.

Infection

Under 1% with strict aseptic technique and prophylactic antibiotics.

Is the BBL safe?+

When performed in the subcutaneous plane only, with appropriate technique and monitoring, the safety profile is now significantly improved versus historical data. We do not perform sub-fascial injection.

How long does the result last?+

The fat that survives the first six months is permanent and behaves like the patient's own native fat — it gains and loses with the body weight.

Will I need a second session?+

Most patients are satisfied with a single procedure. A second session is offered to slim patients who require a staged approach.
